Please read and sign below to grant this permission, indefinitely. This permission may be revoked by you, in writing, at any time, by sending a written letter to: Manager of Compliance Training and Monitoring, CareerSource Palm Beach County, 3400 Belvedere Road, West Palm Beach, FL 33406.
I hereby grant CSPBC permission to use my name, likeness and voice (still and digital images and videos), correspondence and testimonials (written, recorded and virtual), in whole or in part, on CSPBC’s website, affiliated social media sites (including, but not limited to, Facebook, Twitter, YouTube, Instagram and Pinterest), in internal and external publications or for use in any manner deemed appropriate by CSPBC to publicize and promote its programs and activities. This includes release of said information to the media for news and promotional purposes. CSPBC reserves the right to edit written or posted comments/letters and/or otherwise alter the visual or sound recording or still and digital photographs as needed. I understand that I have not been promised nor will I receive compensation for the use of such cited materials. I also hereby release CSPBC and its agents and employees, from all claims, demands and liabilities whatsoever in connection with the above.
